Request documents related to an Iowa driver's license record or traffic ticket
Order your driving record online
Get a certified copy of your driving record on our myMVD website. A certified driving record is your official driving record certified by the Iowa DOT and can be used for official or legal purposes. The fee is $5.50 plus a $3 charge which includes a $1.50 online service fee and $1.50 credit card processing fee. You will need your Iowa driver’s license number and social security number to complete the online request.
If you simply want to view your Iowa driving history or print a noncertified copy, you can do that for free through the myMVD website.
Order a driving record by mail
- Complete the Privacy Act Agreement For Request of Motor Vehicle Records form Please clearly state the type of record you are requesting. If you are requesting this information for yourself, you only need to complete Section A, Section B, and Section D.
- After completing the form, print and sign the request form. Be sure your full name is included at the top of all the printed sheets.
- If you are requesting the record of another person, you must get their written consent and include it with your request form. The Federal Driver’s Privacy Protection Act prohibits the disclosure of personal information about any individual from a motor vehicle record to the public without written consent of the person to whom the information pertains.
- Make a legible photocopy of your driver's license or non-driver ID card.
- Payment must be included with the request. The cost is $5.50 per record.
- Mail completed form, photocopy of driver's license or ID, written consent if requesting the record of another person, and and a check for the appropriate amount made payable to "Treasurer, State of Iowa" to:

Once we have completed your request, we will send the information to the requestor via secure email to the email address listed on the request form.
Order other documents related to an Iowa driver's license record or your accident report
Get a copy of a traffic ticket(s), previously issued driver's license applications showing license history in Iowa, your personal accident report, or any other document related to your Iowa driver's license record.

Accident reports
Reporting an accident
The driver involved in an accident occurring anywhere within the State of Iowa causing death, personal injury, or total property damage of $1,500 or more should contact the nearest law enforcement agency to have the accident investigated. If a law enforcement agency does not investigate the accident, it must be reported on an Iowa Accident Report form. Failure to return an accident report form within 72 hours from the time of the accident may result in suspension of your driving privileges. Note: You must attempt to completely fill out this report.
Also, the insurance information on page four of the report must be completed. Failure to complete insurance coverage information may result in the suspension of your driving and registration privileges.

Obtaining a copy of an accident report
Officer's report
A copy of the officer's report of an accident costs $4. State law allows these reports to be given only to persons involved in the accident, their attorney, insurance companies or insurance agents. To obtain a copy of the report:
- Complete the Request for Copy of Investigating Officer’s Report of an Accident Form
- Print and sign the form.
- Mail the completed form and $4 to:

Obtaining limited information of an accident
A $4 fee is required to obtain a limited information accident report.
